Nevermore was formed after Warrel Dane and Jim Sheppard's previous band, Sanctuary, had disbanded.

Jeff Loomis tried out for Megadeth after Chris Poland was fired in 1987 but didn't get the part as Dave Mustaine didn't think he was ready for the part (his age and lack of touring experience being the biggest factors). Loomis went on to join Sanctuary as a touring guitarist just before they folded and then formed Nevermore with Dane and Sheppard.

Nevermore ceased activity in 2011 after Loomis and Van Williams both departed following disputes with Sheppard and Dane. The latter two then returned to Sanctuary on a full-time basis. Warrel Dane later stated that Nevermore was not definitively finished. This was contradicted in a September 2017 interview with Jeff Loomis, where he confirmed that the band was broken up, but simultaneously acknowledged that a future reunion would be possible since the band members were still on speaking terms with each other. Dane's death later that year, however, seemed to signal a permanent end to the band.

In December 2024, Jeff Loomis and Van Williams announced a reformation of the band without the participation of Sheppard, with whom they had remained estranged since the split in 2011.

New members were found in early 2025 following open auditions, with bassist Semir Özerkan and vocalist Berzan Önen selected from hundreds of applicants. Guitarist Jack Cattoi, on the other hand, was hand-picked by Loomis. The new line-up was publicly announced in February 2026.
