명곡 "Painkiller"를 담고 있는 앨범으로 1990년 9월에 릴리스되었다. "British Steel", "Screaming for Vengeance" 앨범에 이은 헤비메탈 역사의 찬란한 금자탑으로 기록된 본작 "Painkiller"는 Judas Priest 역사상 가장 헤비하고 가장 뛰어난 연주력을 자랑하고 있다. Glenn Tipton과 K.K. Downing의 환상적인 트윈기타 시스템 하에 샤우팅의 모범답안이라 할 수 있는 Rob Halford의 금속성 보이스가 빛을 발하고 있으며 Ian Hill의 안정감 있는 베이스 연주와 새로 영입한 Scott Travis의 한층 업그레이드된 비트의 홍수를 접할 수 있다. 멤버들의 나이가 40대로 접어드는 시기에 발표되었음에도 꿈틀거리는 역동성이 돋보이고 있으며 특히 두 기타리스트는 스윕 아르페지오 속주까지 구사해 후배들의 장점들을 나름대로 수용하려는 자세를 보여주고 있다. "Between the Hammer & the Anvil", "A Touch of Evil", "One Shot at Glory" 등은 개인적으로 가장 좋아하는 곡들로서 영원히 뇌리에 박혀있을 것이다. 이런 엄청난 역작을 발표하고 Rob Halford가 왜 주다스 프리스트를 떠났는지 정말 아쉽게 느껴지는 대목이다. Chris Tsangarides와 Judas Priest의 공동 프로듀싱으로 제작되었다. 가장 위대한 헤비메탈 밴드의 가장 위대한 앨범.

명실상부 헤비메탈 대표음반, 지극히 메탈스럽지만 캐치한 맛이 조금 부족하다, 지나치게 건조하게 몰아치는 사운드가 조금 부담스럽다, 하지만 페인킬러만 놓고봤을 때는 100점을 줘도 부족하다,
